Scattered reports have found that sinus fungus ball (SFB) may progress to chronic invasive fungal rhinosinusitis (CIFRS), but the mechanism is not yet understood. Distinguishing between SFB and CIFRS sometimes tricky. The features of make symptoms imaging findings indistinguishable from those malignant tumors. We herein report a case in which patient was examined treated very early transition CIRFS. An 81-year-old woman with surgery Voriconazole. Computed tomography (CT) showed typical high-intensity mass maxillary bony hypertrophy unusual erosion. Magnetic resonance (MRI) clear contrast edematous mucosa CIFRS. diffusion coefficient (ADC) value particular limited portion lesion. ADC helpful as reference for evaluating
